สิทธิประกันสังคม (สปส.) แย่กว่า บัตรทอง
สปส.แย่กว่าบัตรทอง วันพุธที่ 21 ตุลาคม พ.ศ. 2552 สำนักงานประกันสังคมยอมรับความจริงบริการแย่กว่าบัตรทอง ที่ประชุมคณะกรรมการ (บอร์ด) ประกันสังคม (สปส.) มีมติตั้งคณะอนุกรรมประเมินสถานะกองทุนประกันสังคม เพื่อนำไปปรับปรุงการให้บริการตามที่นายอภิสิทธิ์ เวชชาชีวะ นายกรัฐมนตรี ได้ให้นโยบายเรื่องการขยายสิทธิประกันสังคมของสปส. ไปสู่คู่สมรสและบุตร รวมถึงให้ยกระดับการรักษาพยาบาลให้เทียบเท่ากับสำนักงานหลักประกันสุขภาพแห่งชาติ (สปสช.) หรือบัตรทอง นายสมชาย ชุ่มรัตน์ ปลัดกระทรวงแรงงาน เปิดเผยว่า ขณะนี้บอร์ดสปส. กำลังพิจารณายกระดับการบริการทางการแพทย์ให้เทียบเท่ากับสปสช.อย่างเร่งด่วน โดยคณะอนุกรรมการประเมินสถานะกองทุนประกันสังคมที่ตั้งขึ้น จะพิจารณาว่าควรต้องเพิ่มระดับคุณภาพการบริการใดบ้าง เพื่อให้ผู้ประกันตนพึงพอใจมากขึ้น ทั้งนี้ ได้รวมการปรับปรุงการจ่ายเงินแบบเหมาจ่ายเพื่อให้มีคุณภาพดีขึ้น รวมถึงเรื่องสิทธิประโยชน์ต่างๆ โดยคณะอนุกรรมการชุดนี้ จะต้องคำนวณงบประมาณว่าจะต้องเพิ่มขึ้นอีกมากเท่าไหร่ เพราะเงินกองทุนสปส. มาจากผู้ประกันตน นายจ้าง และรัฐบาล อย่างไรก็ตาม ในการประชุมบอร์ดครั้งนี้ นางสุพัชรี มีครุฑ ผู้อำนวยการกองประสานการแพทย์สปส. ได้เสนอให้ที่ประชุมพิจารณาการบริการทางการแพทย์ โดยได้มีการเปรียบเทียบการให้บริการระหว่างสปส.กับสปสช. ซึ่งพบว่ามี 4 กรณีที่สปส.ให้สิทธิน้อยกว่าสปสช. ประกอบด้วย 1.โรคเดียวกันที่ต้องรักษาตัวในสถานพยาบาลเกินเวลา 180 วันใน 1 ปี ปัจจุบันกำหนดว่า หากเกิน 180 วัน ผู้ประกันตนจะต้องจ่ายเงินเอง แต่สิทธิของสปสช. แม้จะไม่ได้รับการคุ้มครองโดยตรง แต่มีข้อยกเว้นว่าถ้ามีข้อบ่งชี้ว่าจำเป็นต้องรักษาเกิน 180 วัน ก็จะได้รับการ คุ้มครอง 2.การรักษาพยาบาลกรณีทุพพลภาพสปส. กำหนดว่าผู้ประกันตนสามารถเบิกกับประกันสังคมได้ไม่เกินเดือนละ 2,000 บาท ขณะที่สปสช. ใช้คำว่าผู้พิการสามารถใช้สิทธิการรักษาโดยไม่ต้องเสียค่าพยาบาลแต่อย่างใด โดยโรงพยาบาลก็จะนำมาเบิกกับสปสช.อีกครั้ง 3.กรณีเจ็บป่วยฉุกเฉินสปส. มีข้อจำกัดว่าในกรณีฉุกเฉินว่าเบิกได้ไม่เกินปีละ 2 ครั้ง แต่กรณีอุบัติเหตุหรือประสบอันตรายไม่จำกัดจำนวนครั้ง ส่วนกรณีของ สปสช. พบว่ามีการรวม 2 กรณีรวมกัน โดยเมื่อต้นปี 2552 ได้ขยายว่า กรณีฉุกเฉินที่ประชาชนไม่สามารถรักษาที่กำหนดได้ ก็สามารถไปที่โรงพยาบาลอื่นได้โดยไม่จำกัดจำนวนครั้ง 4.กรณีทันตกรรมซึ่ง ปัจจุบันพบว่าสปส. เข้ารับการรับการรักษาโดยให้สิทธิ 4 เรื่อง ได้แก่ เรื่องของอุดฟัน ถอนฟัน ขูดหินปูน ฟันเทียม อะครีลิก แต่เรื่อง 3 ข้อข้างต้นให้เบิกได้ไม่เกิน 250 บาท แต่สปสช.รักษาได้ไม่จำกัด และไม่จำกัดจำนวนครั้ง โดยทั้งหมดนี้คณะกรรมการแพทย์สปส. ยังปรับปรุงไม่แล้วเสร็จ - - - - - - - - - - - - - - - - - - พันตรีศิริชัย ทรัพย์ศิริ นายกสมาคมคนพิการทางการเคลื่อนไหวสากล โทร. 02-990-0331 http://www.apdi2002.com http://www.youtube.com พิมพ์ apdi. หรือ สมาคมคนพิการ 2110520922

นายไพฑูรย์ แก้วทอง รัฐมนตรีว่าการกระทรวงแรงงาน เป็นประธานเปิดการประชุมวิชาการประกันสังคมประจำปี 2552 หัวข้อ “ประกันสังคม วิถีแห่งสุขภาพในปี 2010” พร้อมระบุว่า การประชุมวิชาการในครั้งนี้ มีเป้าหมายเพื่อแลกเปลี่ยนความคิดเห็น เผยแพร่งานวิชาการ รวมถึงงานวิจัยของสำนักงานประกันสังคม (สปส.) พร้อมนำความรู้และแนวคิดไปขยายผลเพื่อสร้างสรรค์สังคมที่มีคุณภาพ ขณะเดียวกันได้เน้นย้ำให้ สปส.หาวิธีปรับปรุงการบริหารจัดการกองทุนฯ ให้ผู้ประกันตนได้รับสิทธิประโยชน์และประสิทธิภาพในการรักษาพยาบาลมากขึ้น โดยไม่กระทบกับสถานะของกองทุนฯ เช่น ผู้ประกันตนต้องได้รับสิทธิในการรักษาพยาบาลไม่น้อยกว่าบัตรทองของสำนักงานหลักประกันสุขภาพแห่งชาติ (สปสช.)

       อย่างไรก็ตาม มีเรื่องที่น่าเป็นห่วง คือ กองทุนประกันการว่างงานที่ล่าสุด เกิดความล้มเหลวในหลายประเทศ สาเหตุเพราะไม่สามารถควบคุม และตรวจสอบผู้ที่กลับเข้ามาทำงานใหม่ แต่ยังคงขอรับเงินประกันการว่างงานได้ดีพอ ซึ่งได้สั่งให้ สปส.ตรวจสอบดูแลอย่างใกล้ชิด

       ด้าน นพ.สนธยา พรึงลำภู ที่ปรึกษาทางการแพทย์ สำนักงานประกันสังคม เปิดเผยข้อมูลจากงานวิจัยเรื่อง “การวิเคราะห์และทบทวนระบบการจัดการ ค่าบริการทางการแพทย์ กรณีภาวะเสี่ยงในกองทุนประกันสังคม” พบว่าค่าบริการทางการแพทย์จากกลุ่มโรคเรื้อรัง 25 โรค เช่น โรคมะเร็ง เบาหวาน โรคความดันโลหิต โรคเอดส์ มีแนวโน้มเพิ่มสูงขึ้นอย่างน่าห่วง โดยในปี 2548 ผู้ประกันตนที่ป่วยเป็นโรคเหล่านี้ มีประมาณร้อยละ 5 ของผู้ป่วยทั้งหมด แต่ สปส.ต้องรับภาระค่าใช้จ่ายสูงถึงร้อยละ 10 ของค่ารักษาพยาบาล กระทั่งในปี 2552 ค่ารักษาพยาบาลเพิ่มขึ้นเป็นร้อยละ 20 และมีแนวโน้มสูงขึ้นอีกในอนาคต ซึ่ง สปส.จะต้องรีบวางแนวทางแก้ไข โดยทุ่มงบประมาณไปที่การป้องกันให้มากขึ้น
